Once your form is signed, visit the Government of Canada EI Sickness Benefits page and follow their step-by-step application process to submit it.

Only if your leave needs to be extended. If you require more time off than what is listed on your current medical certificate, please book an appointment to obtain an updated certificate with new dates.
