You will have a virtual/online video appointment with a Phzio provider. They will assist you with filling out the EI medical certificate. Please be prepared to answer questions regarding your medical condition and reason for medical leave. You will receive your form via email (PDF format) within 24 hours of your appointment start time.

Your Medical Certificate for Employment Insurance Sickness Benefits needs to be submitted to Service Canada. Please note that you may be required to submit additional EI-related paperwork or medical documentation to support or validate your claim. These can be obtained by reaching out to the healthcare provider who has been treating you while on leave. Phzio cannot help with these requests.

To ensure your certificate (EI paperwork for EI sickness benefit) is completed smoothly, please make sure you have your home address, date of birth, phone number, and Social Insurance Number (SIN) on hand when booking your appointment.

Simple errors can be corrected and your EI form can be reissued to you at no cost. You will be able to flag errors and your signing provider will fix the form. If you need a reassessment or a leave extension, there will be a fee and you will need to schedule a new appointment.
Only if your leave needs to be extended. If you require more time off than what is listed on your current medical certificate, please book an appointment to obtain an updated certificate with new dates. There will be a fee associated with this.

You will receive your fully completed and signed Medical Certificate within 24 hours of your appointment so you can prevent delays and check your Service Canada EI claim status sooner. Please note you may experience short delays over a weekend or statutory holiday.
Once your EI form has been received, visit the Government of Canada EI Sickness Benefits page to submit your EI medical certificate online. Follow their step-by-step application process to finalize your claim.
While we do have availability on weekends, these time slots fill up fast. Please do not leave the appointment until the last minute.
Phzio does not replace the treatment you have been receiving for your medical condition. Please note that while our team can help you fill out your EI medical certificate, they will not prescribe any medications or order any tests for you and will not take over the role of treating provider.
If you cancel at least 24 hours before your appointment is scheduled to start, Phzio will reimburse you the full amount and can help you reschedule. If you cancel within 24 hours of your appointment time or you do not show up to the appointment, you will not be reimbursed.
